EMERGENCY INCIDENT RESPONSE

  1. Ensure the situation is safe to access injured parties
  2. Administer immediate first aid, ring for ambulance where appropriate or attend medical centre
  3. Contact emergency services (if required)
  4. Notify Silverback supervisory personnel, and

  1. Complete an incident report form by clicking THIS LINK
  2. Send a copy of the completed incident report to Silverback Pty Ltd by clicking THIS LINK 
  3. FOR SERIOUS INCIDENTS ONLY: If the incident is a defined “serious incident”, READ DEFINITION HERE FIRST, proceed to step 4.
  4. If the incident is serious, click HERE  to complete the DTEIR WH&S Incident notification form
  5. Send the form to DTEIR within 24 hours of the incident
  6. Upload a copy of the completed form to the INCIDENT REGISTER LOCATED HERE
